A dental group with four locations was losing roughly 15 to 20% of booked appointments to no-shows. Patients book, life happens, they forget. The front desk was manually calling patients the day before, which worked when the schedule was light but fell apart during busy weeks.

They were not looking for AI. They wanted fewer no-shows.

What we built

We built a WhatsApp agent that sends a reminder 48 hours before the appointment, waits for a reply, and handles the three things that actually happen: confirmation, reschedule request, or silence.

If the patient confirms, nothing else happens. If they ask to reschedule, the agent collects their available times and passes it to the front desk as a formatted message ready to act on. If there is no reply after 24 hours, it sends one more nudge.

No chatbot menu. No "press 1 for yes." Just a short message in plain Italian that reads like it came from a human at the front desk.

What changed

The front desk now handles two types of tasks instead of three: actual patient questions and the reschedule requests the agent flags for them. The calling part is gone.

The second effect was less obvious but just as valuable: the front desk stopped starting every day with a round of calls nobody liked making. That 45-minute morning block freed up for actual patient intake.

One thing worth saying honestly

This only works if the booking system has a usable API or data export. Two of the four locations were running on different software, so we had to build a small sync layer before anything else. That added two weeks to the project.

If your clinic runs on Doctoralia, Medesk, or any major platform, the setup is straightforward. If it is a custom system from 2009, we will need to look at it first.

Who this applies to: Any appointment-based business where no-shows cost real money. Dental clinics, physiotherapy, beauty, legal consultations. If you have a booking system and a WhatsApp Business number, this is buildable.
